Text: Deceased was found lying unconscious between the empty and full
roads of an endless rope haulage. Apparently he had been in the act of
coupling two full tubs together, when they closed and crushed his head,
which was severely injured. He died very shortly afterwards. It was no part
of his duty to couple the tubs, and 10 minutes previously he had been warned
that he must not attempt to do so.
